FOR ALL DOT PHYSICALS:

-Please call us to schedule an appointment, or to ensure we have a certified provider that is on duty to perform your exam. 
-Please ensure you are aware of the new federal guidelines established 2024. 
-If you are unsure about guidelines or have questions about qualifications, please give us a call so we can better assist you.

 

PRICING INFORMATION

Office visits start at $160, additional services during office visits will be an additional fee. 
CDL's and MSPA's: Out of pocket same day payment start at $95.

* We observe the following holidays; New Years Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, and Christmas Day.

  • Loss of consciousness

  • Severe abdominal pain
